Does anyone have a serious and educated guess what a ptolemaic d20 might really have been used for?

A game of some sort, I'd imagine (DnD jokes aside). Another option might be divination.

My guess would be some kind of game and possibly divination.

Divination is certainly a possibility.

Joachim Quack mentions dice briefly in an article called 'A black cat from the right, and a scarab on your head: New sources for Ancient Egyptian divination', which was published in a book called "Through a Glass Darkly: Magic, Dreams and Prophecy in Ancient Egypt", ed. Szpakowska.

Other dice have been found in the context of divination, and there even seems to be some instructions for it included in a particular papyrus (Vienna D 12006.) Dice-based divination instructions are also known from Mesopotamia.
